Description

Job Title: Nurse Practitioner – In-Home Health Risk Assessment

Location: Gwinnett County, GA (Travel required within a 50-mile radius)

Job Type: Contractor / Per-Diem

Job Summary

We are seeking a qualified Nurse Practitioner (NP) to conduct annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act (ACA) members. These in-home assessments provide valuable insights into members' health needs and risks.

Hours & Compensation

  • Availability: Monday–Friday, 7:00 AM – 7:00 PM (flexible scheduling)
  • Appointments: 25–30 visits per week, each lasting 60–75 minutes
  • Compensation: Market rate per completed assessment
  • Mileage Reimbursement: After the first 30 miles driven per day at the IRS rate

Essential Responsibilities:

  • Complete in-home preventive health risk assessments for eligible health plan members
  • Accurately document clinical findings and submit chart notes within 24 hours
  • Address charting queries within 48 hours
  • Maintain clear and professional communication with clinical leadership staff
  • Adhere to health plan standards to ensure high-quality assessments
  • Attend periodic training sessions and team meetings
  • Comfortably enter members’ living spaces to complete evaluations
  • Physically able to bend, kneel, and lift up to 20 lbs when required

Qualifications:Education & Experience:

  • Master’s or Doctorate Degree from an accredited NP program (FNP, AGNP, ANP) or PA program
  • Home health experience required within the last 5 years
  • 1+ years of in-home assessment experience preferred

Licensure & Certification:

  • Active RN and NP or PA licensure in the state of practice
  • Board certification through ANCC, AANP, or NCCPA
  • Current Medicaid ID and Medicare PTAN Number
  • BLS certification
  • Valid state driver’s license & auto insurance

Key Competencies:

  • Strong proficiency with electronic medical records (EMR)
  • Familiarity with CMS and ACA risk adjustment models
  • Clear and effective verbal & written communication skills
  • Ability to make clinical decisions while following company policies
  • Flexibility and adaptability in a dynamic work environment
  • Maintain patient confidentiality in compliance with HIPAA regulations
